Westat staff will be on site at the 2019 American Academy of Home Care Medicine (AAHCM) Annual Meeting, October 18-19, 2019, in Rosemont, Illinois. We work with AAHCM to evaluate the impact of home-based primary care by creating interactive practice-level profiles. Profiles assess patient complexity and practice performance to identify best practices to improve health outcomes and inform policy decisions.
Westat researchers Joanna Kubisiak and Emily Kubisiak will be hosting Table 21 at the meeting. Attendees will be able to have hands-on interaction with the profiles. We will show how changing certain variables affects overall practice assessment.
